INSTRUCTIONS: WASHINGTON LOCAL SALES & USE TAX ADDENDUM
Washington state businesses are required to file a local sales and use tax form in the county where they operate. If the form provided does not provide enough space to list all sales and use tax fees which must be paid, you may attach the local sales and use tax addendum form. This will be the case if you operate businesses in multiple counties. This form is found on the website of the Washington State Department of Revenue.
Washington Local Sales & Use Tax Addendum Step 1: At the top right-hand corner, enter the period for which you are filing.
Washington Local Sales & Use Tax Addendum Step 2: Below this, enter the year for which you are filing.
Washington Local Sales & Use Tax Addendum Step 3: Enter your name on the line where indicated and your tax registration number to the right of this.
Washington Local Sales & Use Tax Addendum Step 4: The chart provided on the first page extends to the second page. On each line, enter the location code of the transaction and the taxable amount of the sale. Group sales tax and use tax applicable transactions separately.
Washington Local Sales & Use Tax Addendum Step 5: To calculate the deferred sales tax or use tax which is owed, you must visit the website of the Department of Revenue, which lists the percentage of taxation rates for each county.
Washington Local Sales & Use Tax Addendum Step 6: Under the column headed "Local Rate," enter the rate of taxation in the county in which the transaction took place.
Washington Local Sales & Use Tax Addendum Step 7: Multiply the taxable amount by the percentage of the tax rate on each line to determine the tax due to the city or county.
Washington Local Sales & Use Tax Addendum Step 8: Total the tax due from all lines and enter the sum at the bottom of the page. If you are completing the second page as well, enter the total from that page as well.
Washington Local Sales & Use Tax Addendum Step 9: The sum of taxable amounts of sales must be equal to the total taxable amount from State Retail Sales Tax on your tax return form.
Washington Local Sales & Use Tax Addendum Step 10: Attach this addendum to your tax return and file both documents.
