Arizona organizations wishing to organize a special bonus game when operating bingo games must submit a form 71-5411. This application can be obtained from the website of the Arizona Department of Revenue. In order to be eligible to conduct a special bonus game, you must already have obtained a Class B or a Class C bingo license. Note that no more than five special bonus games may be conducted within a quarter and that only one special bonus game may occur on the same respective day of each calendar week.
