INSTRUCTIONS: MARICOPA COUNTY GENERAL ONSITE APPLICATION

To request an inspection of a Maricopa County site by the official water and waste management division, the application discussed in this document should be submitted. This document can be obtained from the official website maintained by the government of Maricopa County.

Maricopa County General Onsite Application Step 1: Indicate with a check mark whether you wish to have a site investigation, a site and test hole inspection, a miscellaneous review or reconnect plan review, or a septic systems abandonment or closure. For the latter two categories, enter your existing permit number.

Maricopa County General Onsite Application Step 2: On the first blank line, enter your property address.

Maricopa County General Onsite Application Step 3: On the second blank line, enter your zip code.

Maricopa County General Onsite Application Step 4: On the third blank line, enter your cross streets.

Maricopa County General Onsite Application Step 5: On the fourth blank line, enter your parcel number.

Maricopa County General Onsite Application Step 6: On the fifth blank line, enter your subdivision name if applicable.

Maricopa County General Onsite Application Step 7: On the sixth blank line, enter the number of lots.

Maricopa County General Onsite Application Step 8: On the seventh, eighth and ninth blank lines, enter your section, township and range.

Maricopa County General Onsite Application Step 9: On the tenth blank line, enter your acreage.

Maricopa County General Onsite Application Step 10: Indicate whether this is a single family residence or a commercial establishment.

Maricopa County General Onsite Application Step 11: Provide all information requested about who will be providing water service.

Maricopa County General Onsite Application Step 12: The next section concerns the property owner and agent. On the first six blank lines, enter the owner's name, complete mailing address, zip code, phone number, fax number and email address. The last two pieces of information are optional.

Maricopa County General Onsite Application Step 13: The next eight lines concern the agent. Enter their name, complete mailing address, zip code, phone number, fax number, mobile phone number and email address.

Maricopa County General Onsite Application Step 14: The applicant should sign and date the bottom of the form.

INSTRUCTIONS: ALABAMA APPLICATION FOR FARMLAND PROTECTION PROGRAM CONSERVATION EASEMENT 

 

 

Alabama agriculture and industries may use the form discussed in this article to apply for farmland protection and conservation provided their land must be at least 50% open, and that at least 50% of the offered farmland is prime farmland. Open farmland is defined as land which is used for pasture, hayland or crops. You may also file this form if you have nationally or state registered historical or archeological sites on the property. You may wish to contact your local Natural Resources Conservation Service office for information on which specific soils on your farm that qualify as prime farmland soils.

Land located nearer to major metropolitan areas and high growth counties will receive higher priority status for funding.
