INSTRUCTIONS: RHODE ISLAND SALES AND USE TAX EXEMPT PURCHASE CERTIFICATION

 

 

Rhode Island sales of tangible personal property to the state government, or a city or town government, as well as to exempt non-profit organizations, qualified redevelopment agencies and dam management districts qualify for an exemption of payment of sales and use tax. Such transactions must be separately detailed with a tax exempt purchase certification form. Sales to state or local governments in other states are not eligible for this exemption. This form can be found on the website maintained by the division of taxation of the state of Rhode Island. 

 

Rhode Island Sales And Use Tax Exempt Purchase Certification Step 1: The top of the form is to be completed by the purchaser. On the first line, their name should be printed or typed, along with their signature.

 

Rhode Island Sales And Use Tax Exempt Purchase Certification Step 2: On the second line, give the name of the exempt organization and its exemption number, as well as the title of the person completing the form.

 

Rhode Island Sales And Use Tax Exempt Purchase Certification Step 3: On the third line, give the street address, city, state and zip code of the business.

The first question should be completed if the transaction was a federal credit card purchase. In such cases, the first four digits of the card number and its sixth digit should be given.

 

Rhode Island Sales And Use Tax Exempt Purchase Certification Step 6: The second section should be completed to cover all other transactions. Give the last four digits of a credit card if used in a transaction. Otherwise, this section affirms that you were paid by check.

Rhode Island Sales And Use Tax Exempt Purchase Certification Step 8: The form should be filed with the seller and maintained in their records. This document does not need to be filed with the state.
