A power of attorney form grants another person to perform actions with legal consequences on your behalf. This Minnesota Power Of Attorney form (REV184) specifically concerns all tax matters processed by the Minnesota Department of Revenue, including non-tax collection efforts referred to this agency by other government divisions. Power of Attorney REV184 Step 1: Enter your name and your Social Security number, Minnesota Tax ID number or federal ID number. If filing jointly with a spouse, they should provide this same information in the line below. Enter your address.

An "original" power of attorney means you are granting the representative designated below this authority for the first time. If you are filing an "amend" power of attorney, this means you are altering the terms of their powers.
