INSTRUCTIONS: MINNESOTA POWER OF ATTORNEY (Form REV184)

 

A power of attorney form grants another person to perform actions with legal consequences on your behalf. This Minnesota Power Of Attorney form (REV184) specifically concerns all tax matters processed by the Minnesota Department of Revenue, including non-tax collection efforts referred to this agency by other government divisions. This document may be obtained from the website of the Minnesota Department of Revenue.

 

Power of Attorney REV184 Step 1: Enter your name and your Social Security number, Minnesota Tax ID number or federal ID number. If filing jointly with a spouse, they should provide this same information in the line below. Enter your address.

 

Power of Attorney REV184 Step 2: You must check from one of the three options provided what kind of power of attorney form this is. An "original" power of attorney means you are granting the representative designated below this authority for the first time. If you are filing an "amend" power of attorney, this means you are altering the terms of their powers. Check "cancel/revoke" if you intend to nullify a previously filed power of attorney with the representative named in this form.

 

Power of Attorney REV184 Step 3: Provide an expiration date where indicated. If you do not do so, this form will take effect indefinitely unless revoked.

Power of Attorney REV184 Step 5: Below this, you can check the box indicating you grant them authority to handle all aspects of tax and nontax debts handled by the Minnesota Department of Revenue.

 

Power of Attorney REV184 Step 6: Check the box below this if you wish to limit their powers in signing tax returns to certain areas. Check the boxes to any items you do not wish for them to be able to sign tax returns for.

 

Power of Attorney REV184 Step 7: If you wish for your representative to receive all correspondence from the department, you must attach a form REV184a to this form.

 

Power of Attorney REV184 Step 8: Print and sign your name, along with the date your phone number. If filing jointly, have your spouse do the same.

 

Power of Attorney REV184 Step 9: Email, fax or mail the form to the Minnesota Department of Revenue.
