Form M13 Income Tax Extension Payment

 

INSTRUCTIONS: MINNESOTA INCOME TAX EXTENSION PAYMENT (Form M13)

 

 

Minnesota residents who are unable to file their tax return by the due date may file a form M13 along with payment for an extension to do so. There is no extension available concerning the actual payment of taxes. This document should be completed on a computer and printed out. This document is available on the website of the Minnesota Department of Revenue. Alternately, you may choose to make your payment on their website for free, which does not require you to mail in this form. You may also submit payment over the phone line listed on the instructions page. A fee will be assessed for this service. Payment must be submitted on or by the tax due date or you will be considered delinquent in your payments.

 

Minnesota Income Tax Extension Payment M13 Step 1: Detach the bottom of the page along the bottom line where indicated. This is the only part of this page that should be mailed in.

 

Minnesota Income Tax Extension Payment M13 Step 2: Enter your first name, middle initial and last name in the space indicated.

 

Minnesota Income Tax Extension Payment M13 Step 3: If you are filing a joint application with your spouse, enter their first name, middle initial and last name in the space indicated.

 

Minnesota Income Tax Extension Payment M13 Step 4: Enter your full street address, including the city, state and zip code.

 

Minnesota Income Tax Extension Payment M13 Step 5: Enter your Social Security number where indicated.

 

Minnesota Income Tax Extension Payment M13 Step 6: If applicable, enter your spouse's Social Security number where indicated.

 

Minnesota Income Tax Extension Payment M13 Step 7: Enter the tax year for which you are filing.

 

Minnesota Income Tax Extension Payment M13 Step 8: Enter the amount of the check which you are sending. 

 

Minnesota Income Tax Extension Payment M13 Step 9: Print out the document. Make sure that your page scaling settings are set at "none." Should they be set at "Auto-rotate and center pages" or "shrink to fit" by default, turn these options off.

 

Minnesota Income Tax Extension Payment M13 Step 10: A printed line of numbers is at the bottom of the page. When you print out this document, this numbers should include the tax-year end date and your Social Security number. Make sure these are properly included.
