INSTRUCTIONS: MICHIGAN PRINCIPAL RESIDENCE EXEMPTION (PRE) AFFIDAVIT (Form 2368)
Michigan home owners may be able to claim an exemption on a portion of their local school operating taxes by filing a form 2368. This form can be found on the website of the Michigan Department of Treasury. Complete the form by typing your response or printing your response in black or blue ink.

Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 3: Give your street address on line 3.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 4: Indicate with a check mark whether you live in a township or city on line 4 and give the name of the village or township. Enter the name of your county in line 5.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 5: On line 6, give the date on which you made this location your primary residence. Indicate with a check mark whether the address given above is your primary residence or an unoccupied adjoining or contiguous property that is classified as timber-cutover or residential.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 6: Give the name of the owner on line 7 and their Social Security number on line 8.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 7: Give the name of the co-owner, if applicable, on line 9, and their Social Security number on line 10.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 8: Give the property owner's daytime phone number on line 11. List the percentage of the property occupied by the owner filing this application on line 12.
Michigan Principal Residence Exemption (PRE) Affidavit 2368 Step 9: Line 13 asks you if you ever claimed a previous exemption for another Michigan homestead. Line 14 asks if you have rescinded this if so. Line 15 asks if you claimed a similar exemption in another state. Line 16 asks if you or your spouse filed a tax return as a non-resident or resident of another state.
