INSTRUCTIONS: COLORADO CLAIM FOR REFUND (Form DR 0137)
Colorado residents and businesses who believe they have overpaid sales tax on an item may file a form DR 0137 with the state Department of Revenue to seek a refund. This form can be found on the website of the state of Colorado. Before filing this request for a refund, you should attempt to obtain a refund from the merchant.
Colorado Claim For Refund DR 0137 Step 1: On the first line, enter the name of the taxpayer.
Colorado Claim For Refund DR 0137 Step 2: On the second line, if you are filing on behalf of a business, enter the name under which the business operates.
Colorado Claim For Refund DR 0137 Step 3: On the third line, enter your mailing address.
Colorado Claim For Refund DR 0137 Step 4: On the fourth line, enter your city, state and zip code.
Colorado Claim For Refund DR 0137 Step 5: On the fifth line, enter your Social Security number and Federal Employer Identification Number.
Colorado Claim For Refund DR 0137 Step 6: On the sixth line, enter your Colorado Department of Revenue account number, the type of tax for which you are seeking a refund (such as state, city or county sales tax) and the beginning and ending dates of the period in question.
Colorado Claim For Refund DR 0137 Step 7: On the seventh line, provide the amount of the original tax paid, the correct amount owed and the amount of the refund you are requesting.
Colorado Claim For Refund DR 0137 Step 8: In the blank space provided, give a written explanation of the reason you are entitled to this refund. If you require additional space, attach a separate piece of paper to continue your explanation.
Colorado Claim For Refund DR 0137 Step 9: Sign the form where indicated. If filing on behalf of a business, the signature must be provided by an officer, partner or firm owner.
Colorado Claim For Refund DR 0137 Step 10: Give your title, telephone number and the date.
Colorado Claim For Refund DR 0137 Step 11: If a paid preparer completed the form, they must give their signature, the name of their firm, their telephone number and the date.
Colorado Claim For Refund DR 0137 Step 12: Submit your form along with all documentation substantiating your claim.
