INSTRUCTIONS: ARIZONA CLAIM FOR REFUND ON BEHALF OF DECEASED TAXPAYER (Form 131)
To request a refund of Arizona taxes overpaid on behalf of or by a decedent, a form 131 should be filed. This document can be obtained from the website of the Arizona Department of Revenue.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 1: In box 1, enter the decedent's name.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 2: In box 2, enter the date of death.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 3: In box 3, enter the decedent's Social Security number.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 4: In box 4, enter the name of the person claiming the refund.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 5: In box 5, enter the claimant's Social Security number or federal identification number.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 6: In box 6, enter the home street address of the claimant.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 7: In box 7, enter the city, town or post office, state and zip code of the claimant.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 8: In box 8, enter the claimant's relationship to the decedent.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 9: In Part I, check the box next to statement 9a, 9b or 9c as applicable.
Arizona Claim For Refund On Behalf Of Deceased Taxpayer 131 Step 10: Part II should only be completed if you checked box 9c, meaning you are neither a surviving spouse or court-appointed or certified personal representative. On line 10a, indicate whether the decent left a will with a check mark.
