INSTRUCTION: ALABAMA POWER OF ATTORNEY AND DECLARATION OF REPRESENTATION (Form 2848A)

An Alabama Power of Attorney and Declaration of Representation form gives someone the authority to perform certain actions with respect to tax matters on behalf of someone else. The form discussed here is only for tax matters. You can obtain this document, formally known as Form 2848A), from the website of the Alabama Department of Revenue.

Power of Attorney and Declaration of Representative 2848A Step 1: In section one, enter your full name, address, Social Security number, Employer Identification Number (if applicable) and daytime phone number.

Power of Attorney and Declaration of Representative 2848A Step 2: In section two, list the names, addresses and phone/fax numbers of those you with to give this status. You may list up to three people on one form.

Power of Attorney and Declaration of Representative 2848A Step 3: In section three, specify the types of taxes (individual, business, etc.), tax form numbers, and years which you authorize them to be given power over.

Power of Attorney and Declaration of Representative 2848A Step 4: In section four, note the specific powers you are granting. A power of attorney cannot receive tax refund checks or provide a signature on certain tax returns on the basis of this section. In addition to the authority receive tax returns, you may specifically list the other powers you are granting.

Power of Attorney and Declaration of Representative 2848A Step 5: If you wish for the representative to be able to receive refund checks, note their name in section five.

Power of Attorney and Declaration of Representative 2848 A Step 6: In section six, indicate if you want multiple representatives to receive all relevant documents or just the one first listed.

Power of Attorney and Declaration of Representative 2848 A Step 7: If you have filed a previous power of attorney and wish for this to still be in effect, attach a copy. Otherwise, it will be automatically revoked.

Power of Attorney and Declaration of Representative 2848 A Step 8: Taxpayers should sign and date the form. If filing jointly, both husband and wife must sign.

Power of Attorney and Declaration of Representative 2848 A Step 9: Under Part II, the representative(s) should give their signature. They must fall into one of the eight categories listed in this section.
