The form discussed in this article is used by the creditor of a Washington D.C. estate with a claim of over $500 that has not been disbarred or been disqualified to waive protection of any bond filed by an estate personal representative. This form can also be used an adult heir or legatee of a Washington D.C. estate for the same purpose. This form is only for the completion of those filing in a case involving the estate of someone who died on or after January 1, 1981 but no later than June 30, 1995. Additionally, the form should be filed if there is no will waiving the requirement of a bond but you wish to waive this requirement. A version of this form that was updated in 2009 has also since been made available for public use. Both of these documents can be obtained from the website maintained by the District of Columbia Courts system.
