INSTRUCTIONS: ARIZONA AUDIT DISCLOSURE AUTHORIZATION FORM (Form 285A)

 

 

To authorize an appointee to discuss tax matters related to an Arizona audit, a form 285A. This document can be obtained from the website maintained by the Arizona Department of Revenue.

 

Arizona Audit Disclosure Authorization Form 285A Step 1: The first section concerns information about the tax payer. In the first box, enter the taxpayer name.

INSTRUCTIONS: ARIZONA DISCLOSURE CERTIFICATION FORM (Form 285C)

A form 285C is used by someone who has been authorized via a form 285 with power of attorney in Arizona tax matters or via a form 285B to receive confidential documents relating to tax matters. The form 285C certifies to the Arizona Department of Revenue that this person has been authorized legally authorized in either of these capacities. The document can be obtained from the website maintained by the Arizona Department of Revenue.

Arizona Disclosure Certification Form 285C Step 1: Section 1 concerns the taxpayer who has authorized you. Enter their name, their spouse's name if applicable, their present street address, apartment suite number (if applicable), city, town or post office, state, zip code, and daytime phone number including area code.

Arizona Disclosure Certification Form 285C Step 2: Additionally, supply all of the following applicable pieces of information: the taxpayer's Social Security number, their spouse's Social Security number, their employer identification number, and/or their Arizona transaction privilege tax license number.

The Affidavit can also be obtained from the website maintained by the Montana Courts system. The notice should be published at least once a week for three successive weeks in a newspaper of general circulation in each county involved in the case. Should there be no newspaper in a county involved, publication in the newspaper of an adjacent county is acceptable.
