INSTRUCTIONS: CONNECTICUT APPLICATION FOR WAIVER OF FEES (Form JD-CV-120)

 

 

To apply for a waiver of payment of fees that normally apply in civil housing small claims court in Connecticut, a form JD-CV-120 should be filed. This document can be obtained from the website of the Judicial Branch of the State of Connecticut.

 

Connecticut Application For Waiver Of Fees JD-CV-120 Step 1: Enter the name of the case and the docket number, if applicable. 

 

Connecticut Application For Waiver Of Fees JD-CV-120 Step 2: Give the address of the court and indicate with a check mark whether you are giving the judicial district, housing session, geographical area number, or small claims court address.

INSTRUCTIONS: CONNECTICUT APPLICATION FOR AUTOMOBILE DEALER'S OR REPAIRER'S LICENSE (Form K-7)

 

 

To apply for a Connecticut license to run an automobile dealer or repairer business, use a form K-7. This document can be obtained from the website of the government of Connecticut.

 

Connecticut Application For Automobile Dealer's Or Repairer's License K-7 Step 1: Section 1 is to be completed by the applicant. Indicate with a check mark whether you are applying for a new car dealer license, used car dealer license, general repairer license or limited repairer license.

Connecticut Application For Automobile Dealer's Or Repairer's License K-7 Step 9: In the next blank box, enter the makes of vehicles you have a factory franchise to sell at the location.

 

Connecticut Application For Automobile Dealer's Or Repairer's License K-7 Step 10: In the table provided, enter the title, name, home address, date of birth and sex of all owners of an individually owned or partnership owned business. If a corporation, enter this information for principal officers or major stockholders. If an LLC, enter this information for members and managers.

 

Connecticut Application For Automobile Dealer's Or Repairer's License K-7 Step 11: Sign the form and give your title before a notary public. Section 2 should be completed by an authorized city official.
