Alabama government departments file the form discussed in this article to give the state records center notification of their intent to destroy records. This document can be obtained from the website maintained by the Alabama Department of Archives and History.

Alabama Destruction Notice RM-12 Step 6: Indicate with a check mark whether the Records Center Supervisor or Agency Records Liaison should be contacted if the Alabama Department of Archive and History objects to the proposed destruction.

Alabama Destruction Notice RM-12 Step 7: Indicate with a check mark whether the records are to be destroyed by the records center or your agency.

Alabama Destruction Notice RM-12 Step 8: Indicate with a check mark whether the records are to be destroyed by shredding, landfill disposal, recycling or other.

Alabama Destruction Notice RM-12 Step 9: Indicate with a check mark whether the records will be replaced with microfilm copies.

Alabama Destruction Notice RM-12 Step 10: On the next blank line, enter your name and title.
