Ohio Fiduciary's Bond 4.2 Step 10: Enter the address of the surety's attorney in fact on the next two blank lines.

 

Ohio Fiduciary's Bond 4.2 Step 11: Enter the net value of real estate owned in this county by the surety on the next blank line.

Ohio Fiduciary's Bond 4.2 Step 13: Enter the signature of their surety on the next blank line.

 

Ohio Fiduciary's Bond 4.2 Step 14: Enter the signature of their attorney in fact on the next blank line.

 

Ohio Fiduciary's Bond 4.2 Step 15: Print or type the name of this attorney in fact on the next blank line. Enter this attorney's address on the next blank line and the net value of real estate owned in this county by the principal on the last blank line.

INSTRUCTIONS: OHIO APPLICATION FOR FAMILY ALLOWANCE (Form 7.1)

 

 

The form discussed in this article is used by Ohio fiduciaries administering an estate to request family allowance in the event that there is a surviving spouse and no minor children who are not of the surviving spouse or if there is no surviving spouse and one minor child of the decedent. In such cases, the surviving spouse or minor child is entitled to $25,000 if the decedent died before March 1, 1999 or $40,000 if they died afterwards. This document can be obtained from the website maintained by the Ohio Supreme Court.

 

Ohio Application For Family Allowance 7.1 Step 1: On the first blank line, enter the name of the county in which the probate court is located.

 

Ohio Application For Family Allowance 7.1 Step 2: On the second blank line, enter the name of the decedent whose estate is being administered.

 

Ohio Application For Family Allowance 7.1 Step 3: On the third blank line, enter the case number.

 

Ohio Application For Family Allowance 7.1 Step 4: In the paragraph which follows, check the first statement if there is a surviving spouse and no minor children who are not of the surviving spouse.

Ohio Application For Family Allowance 7.1 Step 6: Check the third statement if you are requesting that the surviving spouse or minor child receive $25,000 in money or property.

 

Ohio Application For Family Allowance 7.1 Step 7: Check the fourth statement if you are requesting that the surviving spouse or minor child receive $40,000 in money or property.

 

Ohio Application For Family Allowance 7.1 Step 8: The table provided below is for documentation of the money or property you are requesting to have passed along. Enter a description of the money or property in the first column.

 

Ohio Application For Family Allowance 7.1 Step 9: Enter the appraised value of each item in the second column.

 

Ohio Application For Family Allowance 7.1 Step 10: On the next blank line, the fiduciary should enter their signature.
