Form 12.902(c) Family Law Financial Affidavit

INSTRUCTIONS: FLORIDA FAMILY LAW FINANCIAL AFFIDAVIT (Form 12.902(c))
A Florida Family Law Financial Affidavit must be completed by parties involved in a divorce and who earn $50,000 or more a year unless a settlement agreement has been negotiated or both spouses have waived this requirement. The form can be obtained from the website of the Florida court system.
Florida Family Law Financial Affidavit 12.902(c) Step 1: At the top of the third page, enter the name of the plaintiff and respondent, the number of the circuit court you are appearing in and its county, and the case number.

Florida Family Law Financial Affidavit 12.902(c) Step 3: Section I concerns all sources of income. These must be computed on a monthly basis. If your income fluctuates, use the instructions on the first page of instructions in order to create a monthly average figure. In addition to all current income information requested in lines 1-17, lines 18-26 request that you enter all deductions, such as wages withheld for taxes and retirement funds. Subtract your deductions from your total income and enter your net monthly income on line 27.
Florida Family Law Financial Affidavit 12.902(c) Step 4: Section II requires you to detail your average monthly expenses. These are broken down by category into household costs, automobile costs, childcare expenses, insurance, payments to creditors and miscellaneous expenses.
Florida Family Law Financial Affidavit 12.902(c) Step 5: Section III requires you to detail your assets and debts. When listing assets, include their current market value and whether you own them jointly with your spouse or solely. Do the same when listing all debts.

Form FA-4139 Financial Disclosure Statement
INSTRUCTIONS: WISCONSIN FINANCIAL DISCLOSURE STATEMENT (Form FA-4139)
In divorce cases, both parties may be required to submit a financial disclosure statement. This form FA-4139 can be completed by either party in a contested divorce, or either party in a joint petition for an uncontested divorce. The document is available on the website of the Wisconsin Court System.
Wisconsin Financial Disclosure Statement FA-4139 Step 1: At the top of the first page, indicate which party is filing this form. Enter both spouses' names, the case number you have been assigned, and the county in which you are appearing. You must attach your most recent W-2 tax statement and a statement detailing your income for the current year, such as a bank statement.
