INSTRUCTIONS: CALIFORNIA INCOME AND EXPENSE DECLARATION (Form FL-150)

Divorcing couples who are resolving alimony and lawyer’s fees issues are required to individually complete a California Income and Expense Declaration. This Form FL-150 can be downloaded from the California Courts system.

California Income And Expense Declaration FL-150 Step 1: At the top of the form, enter your name and address or that of your attorney, the county and location of the court where your case is being heard, and the name of your other spouse (the “respondent”).

California Income And Expense Declaration FL-150 Step 2: In section 1, provide all information requested about your current place of employment. You should attach copies of all pay stubs from the last two months. People with more than one job should detail all sources of income on a separate blank piece of paper.

California Income And Expense Declaration FL-150 Step 3: Under section 2, write your name and note all education and training you have received.

California Income And Expense Declaration FL-150 Step 4: Under section 3, summarize all requested information about your tax returns.

California Income And Expense Declaration FL-150 Step 5: In section 4, give an estimation of the respondent’s pre-tax monthly income.

California Income And Expense Declaration FL-150 Step 6: Detail your work income in section 5 and your investment income in section 6.

California Income And Expense Declaration FL-150 Step 7: In section 7, detail any self-employment income. In section 8, indicate any other sources of income. If you have recently had a drastic change in your financial status, explain why in section 9.

California Income And Expense Declaration FL-150 Step 8: List all expenses that could count as deductions in section 10.

California Income And Expense Declaration FL-150 Step 9: In section 11, list your property and financial assets.

California Income And Expense Declaration FL-150 Step 10: In section 12, list everyone who lives with you, their relation to you and their income, if any.

California Income And Expense Declaration FL-150 Step 11: In sections 13 through 15, list all debts and expense.

California Income And Expense Declaration FL-150 Step 12: The third page should only be completed by people involved in a child support dispute. Note how many children you have and all related expense.
