Form Ex. P Short Form Financial Declaration

 

INSTRUCTIONS: NEVADA SHORT FORM FINANCIAL DECLARATION (Form Ex. P)

In Nevada divorces in which alimony, child support, lawyer's fees, division of property and other issues are at stake, both parties may be required to submit a financial declaration. This articles concerns the Nevada Short Form Financial Declaration, available on the state Supreme Court's website.

Nevada Short Form Financial Declaration Ex. P Step 1: Enter your name, address and telephone number at the top of the first page. Write the district court number of the court hearing your case and the county.

Nevada Short Form Financial Declaration Ex. P Step 2: Skip to the second page. The first question concerns your monthly income. Those who are unemployed should answer the first two questions. Those who are employed should write "not employed" in response to both.

Nevada Short Form Financial Declaration Ex. P Step 3: Question three requires you to detail your monthly wages.

Nevada Short Form Financial Declaration Ex. P Step 4: Questions four through fifteen require you to enter all other sources of income on a monthly basis. Total your income from wages and other sources to determine your gross income.

Nevada Short Form Financial Declaration Ex. P Step 5: The next questions concern expenses. On the top half of the fourth page, detail all taxes and other expenses withheld from your pay. In the next section, enter all expenses you pay directly.

Nevada Short Form Financial Declaration Ex. P Step 6: The final section concerns all assets and debts that you own jointly and solely. These include homes, stocks, bonds, bank accounts, cash on hand, and retirement accounts. Include the current value of all of these. Detail all vehicles owned, including how much each one is worth and how much must still be paid.

Nevada Short Form Financial Declaration Ex. P Step 7: The first page must be completed, dated and signed in the presence of a notary public once you have completed this financial declaration.

INSTRUCTIONS: CLARK COUNTY COMPLAINT FOR DIVORCE (Packet 16)

In Clark County, Nevada, husbands and wives with children may file for divorce and represent themselves. This article discusses Packet 16, which contains the Clark County Complaint For Divorce along with all other forms necessary to initiate the separation process.

Clark County Complaint For Divorce 16 Step 1: The most important document contained in this package is the actual complaint. This form requires you to provide information about you, your spouses, your children and to detail your wishes regarding custody, visitation rights, division of property and all other issues involved in ending your relationship.

Clark County Complaint For Divorce 16 Step 2: The Family Court Cover Sheet requires the name and address of both parties in the divorce, as well as that of all children, and to indicate the type of divorce you are seeking.

Clark County Complaint For Divorce 16 Step 3: The Domestic Summons sheet will be served upon your spouse once you have submitted your complaint. This will notify them of the legal action which has been taken against them. An Affidavit of Service must be completed by whoever serves the document. You may not serve your spouse yourself.

This document warns against selling joint property, harassing or disturbing you, or abducting any children involved in the marriage.
