Form 12.902(c) Family Law Financial Affidavit

INSTRUCTIONS: FLORIDA FAMILY LAW FINANCIAL AFFIDAVIT (Form 12.902(c))
A Florida Family Law Financial Affidavit must be completed by parties involved in a divorce and who earn $50,000 or more a year unless a settlement agreement has been negotiated or both spouses have waived this requirement. The form can be obtained from the website of the Florida court system.
Florida Family Law Financial Affidavit 12.902(c) Step 1: At the top of the third page, enter the name of the plaintiff and respondent, the number of the circuit court you are appearing in and its county, and the case number.

If your income fluctuates, use the instructions on the first page of instructions in order to create a monthly average figure. Subtract your deductions from your total income and enter your net monthly income on line 27.
