INSTRUCTIONS: NH CERTIFICATE OF LIMITED PARTNERSHIP

 

Use this form to establish a limited partnership in the state of New Hampshire.  You’re strongly encouraged to consult with an attorney while compiling the Certificate and the SRA form. 

 

“NH Certificate of Limited Partnership Step 1”

State the name of the limited partnership on the first line of the Certificate.  Make sure the name contains the words “limited partnership.”  List the physical address of the office on the next set of lines. 

 

“NH Certificate of Limited Partnership Step 2”

List the name of the agent and the address on the next set of lines.  If there is an estimated dissolution date, list the full date on the next line.  If the applicant wants to address anymore matters, attach additional sheets. 

 

“NH Certificate of Limited Partnership Step 3”

All general partners need to sign this form and provide their address as well.  If additional space is needed, attach a separate sheet. 

 

“NH Certificate of Limited Partnership Step 4”

You now need to complete the Addendum to Business Organization and Registration Forms (Form SRA).  In Part I, make sure the business name matches the proposed name at the top of the Certificate.  Include the business address and contact information as well. 

 

“NH Certificate of Limited Partnership Step 5”

You need to mark ONE section in Part II.  Mark the first section if the limited partnership meets criteria under all of the subsections.  Mark the second section if the business qualifies for another exemption or will file for federal covered securities. 

 

Check the third section if the limited partnership has or needs to register the securities for sale in the state.  If you check section 3, make sure to provide the filing date with the Bureau of Securities Regulation.  Only check section 4 if the LP was formed in another state and no securities will be sold in New Hampshire.

Check the appropriate box in Part III.  One or more authorized partners need to sign the bottom of this form.

INSTRUCTIONS: TEXAS CERTIFICATE OF FORMATION LIMITED PARTNERSHIP (Form 207)

 

 

To form a limited partnership in the state of Texas, you will need to submit an application to the secretary of state. You can obtain the appropriate form 207 from the website of the Texas Secretary Of State.

 

Texas Certificate Of Formation Limited Partnership 207 Step 1: In response to article one, give the name of your limited partnership.

 

Texas Certificate Of Formation Limited Partnership 207 Step 2: In response to article two, give the name and address of your registered agent or office. Every limited partnership is required to have a registered agent who has an office registered in Texas at all times. If either the agent or their address changes at any time, a statement should be filed with the office of the Secretary of State in order to amend your certificate of formation. Businesses which do not maintain a registered agent with a Texas registered office may have their certificate terminated.

Form Community Service

INSTRUCTIONS : COMMUNITY SERVICE FORM


A community service form is a document that is used to show completion of community service.  Usually, community service is mandated for criminal or statutory violations instead of jail or probation.  This form is used to prove community service obligations were met.  

In this case, we are using the community service form used by Family Drug Treatment Court in the state of Washington.  Many different examples of community service forms can be found online or through the court in which community service was given.   

The first step in completing the form is to enter the participant’s name and the amount of total hours of community service they are ordered to serve.  


1. A deadline should be provided, informing the participant of when they must turn their community service form in. Fill in this time on each form submitted.


2.  If a drug court date is scheduled, it should be written on the community service form.  


3. The next step is to fill out the grid indicating the time spent on community service.


4. Provide the name of the agency where the community service was completed, their phone number, your supervisor’s contact name, and the date in which you competed the community service. 


5. The agency must sign each instance of community service, providing your sign in time and sign out time. If they have any additional comments, they will write them in the comments section.


6. The participant must then sign and date the form, indicating the town in which they signed it.  


7. Additional pages may be necessary if more community service time was completed than can fit on the grid provided.  If you file additional sheets, ensure that your provide the required information on each individual sheet.  


8. Submit the form to the appropriate case manager and keep a copy for your records.

INSTRUCTIONS: TEXAS APPLICATION FOR REGISTRATION OF AN ENTITY NAME (Form 502)

 

 

Banks, trust companies, savings associations, insurance companies, and foreign filing entities may use a Texas Form 502 to register their name. This document is processed by the Texas Secretary of State and made available on their website. This can be filed by mail or in person at the address provided in the instructions, or faxed to the number given in the instructions.

 

Texas Application For Registration Of An Entity Name 502 Step 1: Prior to completing the form, visit the URL given in the instructions to research whether your name is available. You may also contact the office of the Secretary of State to request a preliminary determination on the availability of your chosen name. Note that a final determination cannot be made until you have filed your form 502.

INSTRUCTIONS: UTAH CERTIFICATE OF CANCELLATION FOR A LIMITED PARTNERSHIP

 

 

In order to begin practicing as a limited partnership in the state of Utah, you are required to file a registration certificate form. Once this document has taken effect, you may wish to dissolve the partnership prior to the end of the term specified on your certificate. If this is the case and all general partners agree to the dissolution, you must file the appropriate form to dissolve a limited partnership in Utah. This article discusses the certificate of cancellation which must be filed with the state's Division of Corporations & Commercial Code. The form, along with all other documents filed concerning your business, is available on their website. If you wish to amend, merge or convert your limited partnership, a different form is made available for this purpose. It is also possible to submit all of these documents online.
