Those who wish to form a limited liability company (LLC) in Washington state will need to complete a certificate application. This document is filed with the Secretary of State. This form is available on that state official's website.
Washington Certificate Of Formation (Limited Liability Company) WA Step 1: At the top of the page, indicate whether you have paid the filing fee for regular or expedited processing.

In addition to the $100 charged for initial processing, an additional $15 is charged for each additional certified copy you request. If you would like a filed, stamped copy returned, include two copies. Otherwise, you need only include one.
