INSTRUCTIONS: ARTICLES OF AMENDMENT TO CHANGE LLC NAME LLC-2

This form is used to change the name of a limited liability company currently functioning within the state of Hawaii.  There are other Articles of Amendment for Hawaii, but this form directly concerns the name change of an LLC.

State the current name of the limited liability company on line 1.  You may want to provide the file number under the Department of Commerce and Consumer Affairs next to the name as well.

“Articles of Amendment to Change LLC Name LLC-2 Step 3”

Provide the new name for the limited liability company on line 2 of this form.  The name of the company needs to contain the phrase Limited Liability Company, L.L.C., or LLC.

A least one member of the limited liability company needs to sign and print their name at the bottom of this form.  Make sure to provide a date of signature in the paragraph above the signatures.
