INSTRUCTIONS: WASHINGTON D.C. AFFIDAVIT OF SERVICE BY MAIL

 

 

The form discussed in this article is completed by the person who served Notice of Hearing on Petition in a Washington D.C. probate case by placing a copy of the notice in an official depository of the United States Postal Service in an envelope with postage prepaid. This document can be obtained from the website maintained by the District of Columbia Courts system.

 

Washington D.C. Affidavit Of Service By Mail Step 1: On the first blank line, enter the name of the adult who is the subject of the case.
