INSTRUCTIONS: APPLICATION FOR RESERVATION OF NAME 635_0051
An applicant can use this form to reserve the name for a profit corporation, nonprofit corporation, limited liability company, limited partnership, or a cooperative association. Before filing out this form, make sure to review the Act applying to the specific type of business in Iowa.

Check the appropriate box in the first section of this form. Check the first box if the name is pursuant to the Iowa Business Corporation Act for profit corporations. Check the second box if the name applies to the Revised Iowa Nonprofit Corporation Act for nonprofit corporations.
You need to check the third box for limited liability companies, the fourth box for a limited partnership, or the fifth box for a cooperative association.

If you checked the fourth box in the first section, you also have to check the appropriate box in the subsection below. Refer to the numbers below for the appropriate box:
1. Mark box 1 if the applicant is the person attempting to organize the limited partnership.
2. Mark box 2 if the applicant is a foreign LP attempting to engage in business in Iowa.
3. Mark box 3 if the foreign LP is trying to obtain a certificate of authority in the state of Iowa.
4. Mark box 4 if the applicant is trying to form a form a foreign LP and wants to obtain a certificate of authority in the state of Iowa as well.
5. Mark box 5 if the applicant is a foreign LP being formed under the same name.
6. Mark the last box if the applicant is a foreign LP forming under a name that fails to comply with §488.108 subsection 2 or 3.

State the name of the company on line 1. Make sure the name complies with state code.

Provide the name and address of the applicant under line 2. The applicant then needs to sign their name, print their name, and date the form.

Provide a filing fee of $10.00 and make a check payable to Secretary of State. Send the form and fee to the following address:
