INSTRUCTIONS: ALABAMA FIREFIGHTER CERTIFICATION ACT 2004-637

 

 

According to Alabama Act 2004-637 permit full-time certified firefighters or law enforcement officers who do not receive state policeman coverage to purchase a Hazardous Duty service credit. This is done by filing the form discussed in this article, which can be found on the website of the Retirement Systems of Alabama.

 

Alabama Firefighter Certification Act 2004-637 Step 1: The first part of the form will be completed by your current employer, then returned to you for filing. On the first blank line, your employer should enter your name.

 

Alabama Firefighter Certification Act 2004-637 Step 2: On the second blank line, your employer should enter your Social Security number.

 

Alabama Firefighter Certification Act 2004-637 Step 3: On the third blank line, your employer should enter your complete address.

 

Alabama Firefighter Certification Act 2004-637 Step 4: On the fourth and fifth blank lines, they should enter your work and home phone numbers.

 

Alabama Firefighter Certification Act 2004-637 Step 5: On the sixth blank line, your employer should enter the title of your position.

 

Alabama Firefighter Certification Act 2004-637 Step 6: Your employer should indicate whether you are full-time or part-time with a check mark.

 

Alabama Firefighter Certification Act 2004-637 Step 7: On the seventh blank line, your employer should enter the name of your employing agency.

 

Alabama Firefighter Certification Act 2004-637 Step 8: The certifying official should sign and date Part I, as well as entering their title.

 

Alabama Firefighter Certification Act 2004-637 Step 9: Part II should be completed by the employer to whom eligible periods of service were rendered. Your position title should be entered in the first column.

Alabama Firefighter Certification Act 2004-637 Step 14: File the form by mailing it to the address given on the second page. Part III will be completed by the Employees' Retirement System of Alabama.
