INSTRUCTIONS: CALIFORNIA CONFIDENTIAL CONSERVATOR SCREENING FORM (Form GC-314)

California Confidential Conservator Screening Form GC-314 Step 1: In the box at the top of the page, enter the name of your attorney, their state bar number, their telephone number and the name of the client being represented. If you do not have an attorney, enter the applicable information for yourself. In either case, including a fax number or email address is optional.
