INSTRUCTIONS: CALIFORNIA STIPULATION TO ESTABLISH OR MODIFY CHILD SUPPORT AND ORDER (Form FL-350)

When two parents who are divorcing or who have separated agree on issues of child support, they may file a form requesting the approval of their terms. This stipulation to establish or modify child support can also be used if you wish to change the terms of an already-approved child support order. The document can be found on the website of the California courts.

California Stipulation To Establish Or Modify Child Support And Order FL-350 Step 1: Enter the address and brand name of the court processing your case. Enter you or your attorney's address at the top of the page and give the names of the plaintiff and respondent. Enter the assigned case number where requested.

California Stipulation To Establish Or Modify Child Support And Order FL-350 Step 2: Question 1 asks you to give both parents' net monthly disposable income. Question 2 asks what percentage of the time each parent has responsibility for their children. If either parent is facing a hardship for which they seek relief, note this in response to question three.

California Stipulation To Establish Or Modify Child Support And Order FL-350 Step 3: In question 4, write in how much child support guidelines say must be paid. If you accept this amount, check the box next to question 5. Otherwise, write what your alternate proposed payment is in response to question 6a.
