INSTRUCTIONS: WYOMING CONFIDENTIAL FINANCIAL AFFIDAVIT

 

 

In all Wyoming family court disputes over child support, both parents are required to submit a financial affidavit. This article discusses this form, which is available on the Wyoming state courts system website.

 

Wyoming Confidential Financial Affidavit Step 1: At the top of the first page, give all identifying information requested about your case. Print your name again where indicated.

 

Wyoming Confidential Financial Affidavit Step 2: Give your full name and gender in section one. Give your current address and telephone number in section two. Give your Social Security number in section three.

 

Wyoming Confidential Financial Affidavit Step 3: In section four, indicate whether you are employed, self-employed or unemployed. In section five, give all information requested about your employer. In section six, detail your job history for the last three years.

 

Wyoming Confidential Financial Affidavit Step 4: In section seven, detail your educational history as instructed.

 

Wyoming Confidential Financial Affidavit Step 5: In section eight, list all children you are responsible for supporting who live with you. Give their names, birth dates, Social Security numbers, and detail the child support orders in effect. 

 

Wyoming Confidential Financial Affidavit Step 6: In section nine, list any children you support who do not live with you. 

 

Wyoming Confidential Financial Affidavit Step 7: List any other children not listed above that you are the parent of in section ten. 

 

Wyoming Confidential Financial Affidavit Step 8: In section eleven, list any children you support towards whom you have no legal obligation.

 

Wyoming Confidential Financial Affidavit Step 9: In section twelve, list all children involved in the current case. 

 

Wyoming Confidential Financial Affidavit Step 10: Answer questions about unpaid support obligations in sections thirteen and fourteen.

 

Wyoming Confidential Financial Affidavit Step 11: List all public benefits granted your children in section fifteen.

 

Wyoming Confidential Financial Affidavit Step 12: Sections sixteen and seventeen require you to detail all income from all sources.

 

Wyoming Confidential Financial Affidavit Step 13: Sections eighteen and nineteen concern health insurance for your children.

 

Wyoming Confidential Financial Affidavit Step 14: Make sure to attach copies of your last two income tax returns, all W-2 forms from the last two years, and statements of earnings from your employer for the last two years.

 

Wyoming Confidential Financial Affidavit Step 15: Sign and date the last page before a notary public or court clerk.

Form 442 Petition For Divorce/Annulment

 

INSTRUCTIONS: DELAWARE PETITION FOR DIVORCE/ANNULMENT (Form 442)

To initiate a divorce or annulment in the state of Delaware, you should file a petition for divorce/annulment. Form 442 can be obtained from the state's official government website. This form can be completed by you or a lawyer at any time, but must be signed before a clerk of court or a notary public.

Delaware Petition For Divorce/Annulment 442 Step 1: Section one, "General information," requires you to provide information about you and your spouse, including the length of your marriage and your addresses. Any children should be listed, along with their dates of birth and addresses.

Delaware Petition For Divorce/Annulment 442 Step 2: Section two should only be completed by those who are seeking divorce. If you are seeking an annulment, skip to step three. In this section, place a check mark next to the box which describes your reasons for seeking divorce. You may claim incompatability, misconduct or mental illness. The latter two require a written explanation. You may also note that this is a voluntary separation without an explanation. Check the box at the bottom of the page stating that you are unlikely to reconcile.

Delaware Petition For Divorce/Annulment 442 Step 3: Section three is only for people who are seeking an annulment. With a check mark, choose the reason why your marriage is invalid from the listed options and provide an explanation of how and when you learned your marriage was invalid.

Delaware Petition For Divorce/Annulment 442 Step 4: In section four, you should note what you want the court to do in addition to ordering a divorce or annulment. You may ask the court to rule on property division and who must pay legal fees, to order alimony fees, to validate a name change following a divorce, or to enforce a separation agreement. A copy of this agreement must be attached to the petition.

Delaware Petition For Divorce/Annulment 442 Step 5: Provide your signature at the bottom of page five.

Delaware Petition For Divorce/Annulment 442 Step 6: On page six, indicate if your divorce is uncontested. If so, you may request that divorce be resolved without a hearing.

Delaware Petition For Divorce/Annulment 442 Step 7: You or your attorney should sign or date the form before a licensed official.
