INSTRUCTIONS: MARICOPA COUNTY AFFIDAVIT OF FINANCIAL INFORMATION (Form DROSC13F)

During divorce proceedings in Maricopa County, Arizona, both spouses must submit an Affidavit of Financial Information in order to help create a separation plan with regards to division of assets, alimony and child support payments.This document can be completed by either party or their legal representative. You can submit this document, known formally as Form DROSC13F, from the website of the Superior Court of Maricopa County.

Maricopa County Affidavit of Financial Information DROSC13F Step 1: At the top of the first page, enter the name of both parties in the case, the number assigned to the divorce case, and the name of the person whose information is detailed in this affidavit.

Maricopa County Affidavit of Financial Information DROSC13F Step 2: Sign and date the document on the first page.

Maricopa County Affidavit of Financial Information DROSC13F Step 3: Every question must be answered. If you do not know the answer to a question, write "Do not know" in that line. Copies of your two most recent pay stubs, federal tax returns for the last three years, and W2 and 1099 forms for the same period must be attached. If not, provide an explanation.

Maricopa County Affidavit of Financial Information DROSC13F Step 7: Sections IV and V concern income from a job and self-employment income, respectively. All forms of compensation must be detailed as requested.

Maricopa County Affidavit of Financial Information DROSC13F Step 8: Section VI is for detailing all expenses related to minor children, while Section VII concerns all other expenses. In both cases, provide a monthly average for every question if the expense in question varies.

Maricopa County Affidavit of Financial Information DROSC13F Step 10: When completed, copies of the affidavit must be submitted both to the presiding judge and the other party in the divorce.

INSTRUCTIONS: HAWAII ASSET AND DEBT STATEMENT (Form 1F-P-063)

After filing a complaint for divorce in Hawaii, one of the documents a plaintiff must submit is an Asset and Debt Statement. This document can be submitted jointly in an uncontested divorce or may be filed separately by both parties.

Hawaii Asset And Debt Statement 1F-P-063 Step 1: At the top of the page, indicate whether the plaintiff, defendant or both spouses are completing the form. Write in the case number.

Hawaii Asset And Debt Statement 1F-P-063 Step 2: Enter the full name of the plaintiff and defendant.

Hawaii Asset And Debt Statement 1F-P-063 Step 3: Indicate which party or whose attorney is completing this document, along with their name, address and phone number.

Hawaii Asset And Debt Statement 1F-P-063 Step 10: In section seven, list all life insurance you have, including the beneficiary and current cash value.

Hawaii Asset And Debt Statement 1F-P-063 Step 12: In section nine, detail miscellaneous assets.

Hawaii Asset And Debt Statement 1F-P-063 Step 13: List all debts in section eleven, and indicate which spouse is associated with each.

Hawaii Asset And Debt Statement 1F-P-063 Step 14: Sign and date the form. Indicate if you are the plaintiff or defendant.
