INSTRUCTIONS: OREGON PETITION FOR DISSOLUTION OF MARRIAGE/DOMESTIC PARTNERSHIP (Form 1B)

To represent yourself in an Oregon divorce involving children, you can file a petition for a dissolution of marriage or domestic partnership. This Form 1B is available on the website of the Oregon Judicial Department.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 1: At the top of the page, enter the county name. Enter your name above "Plaintiff," your spouse's above "Respondent," and the name of any dependant child over 18 and under 21 where indicated.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 2: In line one, enter the location and date of your marriage or domestic partnership. Section three concerns residency for those in a domestic partnership, while section four concerns residency for spouses.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 3: In sections six through eight, provide all information requested about children, including their names and addresses. In section nine, outline your wishes concerning custody, visitation and other parenting plan aspects.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 4: In section ten, provide information about current and proposed child support payments. In section eleven, indicate who should maintain life insurance for their children's sake and for how much.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 5: In section twelve, write out any additional conditions you wish to have imposed. In section thirteen, indicate whether you are not seeking alimony or life insurance claims or have attached page 5a.

Oregon Petition For Dissolution Of Marriage/Domestic Partnership 1B Step 7: In section eighteen, enter whether you wish for your former name or that of your spouse to be restored. Complete section nineteen or explain why doing so would be dangerous.
