INSTRUCTIONS: CONNECTICUT DIVORCE COMPLAINT (DISSOLUTION OF MARRIAGE) (Form JD-FM-159)

During divorce proceedings in the state of Connecticut, either party may file a form JD-FM-159 at the start. If one person files this Divorce Complaint (Dissolution of Marriage) against their spouse, it should be submitted along with a Summons form (JD-FM-3) and Notice of Automatic Court Orders (JD-FM-158). This form can also be filed by the other spouse as a "cross complaint." If this is the case, it should be submitted to the court along with an Answer (JD-FM-160) form. These documents can be obtained from the website of Connecticut's Judicial Branch.

Connecticut Divorce Complaint (Dissolution Of Marriage) JD-FM-159 Step 1: At the top of the page, enter the full names of the plaintiff and defendant, as well as the location where legal action is taking place.

Connecticut Divorce Complaint (Dissolution Of Marriage) JD-FM-159 Step 5: In sections seven through eleven, list all minor children in the marriage, their relationship to each parent, and their date of birth. If any of these children are in the custody of another person or a parent has been ordered to pay support, note the names of both these parties.

Connecticut Divorce Complaint (Dissolution Of Marriage) JD-FM-159 Step 8: In section fourteen, note any city or town financial support provided to parents or children.

Connecticut Divorce Complaint (Dissolution Of Marriage) JD-FM-159 Step 9: Under the words "The court is asked to rule," place an "x" in the box next to every applicable statement. You may ask a judge to rule on any aspect of the divorce in addition to ordering a dissolution of marriage.
