INSTRUCTIONS: OHIO AFFIDAVIT OF INCOME AND EXPENSES

In divorce cases where alimony or child support payments are at issue, both spouses may be required to file an affidavit of income and expenses. This document can be obtained from the website of the Ohio Supreme Court.

Ohio Affidavit Of Income And Expenses Step 1: Enter the name of the county where your case is being heard at the top of the page.

Ohio Affidavit Of Income And Expenses Step 2: Enter the name of the plaintiff and defendant, the assigned case number, and the judge and magistrate hearing your case.

Ohio Affidavit Of Income And Expenses Step 3: Where indicated, print your name. Enter the dates of marriage and separation.

Ohio Affidavit Of Income And Expenses Step 4: In section I, enter all information requested about the employment status and income of husband and wife. Section A concerns both spouses' yearly income, while Section B concerns both spouses' current income.

Ohio Affidavit Of Income And Expenses Step 5: In section II, enter the names, birth dates and the person each minor child in the marriage is currently living with. Enter the number of any other child or adult household members.

Ohio Affidavit Of Income And Expenses Step 6: Section III concerns your expenses. Section A asks you to detail monthly housing expenses, such as rent and utilities.

Ohio Affidavit Of Income And Expenses Step 12: Section IIIG is for other miscellaneous monthly expenses not noted above.

Ohio Affidavit Of Income And Expenses Step 13: Section IIIH is for monthly installment payments, such as car and credit card payments.

Ohio Affidavit Of Income And Expenses Step 14: Add all sub-totals from Sections III A-H in order to determine your total monthly expenses.

Ohio Affidavit Of Income And Expenses Step 15: Sign and date the form before a notary public.
