INSTRUCTIONS: MASSACHUSETTS FINANCIAL STATEMENT SCHEDULE A (Form CJD301A)

In Massachusetts divorce cases involving alimony, child support or other settlement costs, both parties will be required to complete financial statements. This article discusses the Schedule A form, a supplemental document that must be submitted with either a short form or long form Financial Statement by those who are self-employed or own a business. The document can be obtained from the website of the Massachusetts Court System.

Massachusetts Financial Statement Schedule A CJD301A Step 1: At the top of the document, enter your name and the docket number assigned to your case.

Massachusetts Financial Statement Schedule A CJD301A Step 2: Enter your gross monthly income where indicated.

Massachusetts Financial Statement Schedule A CJD301A Step 3: Detail your monthly business expenses as listed. Cost of goods sold, advertising expenses, taxes, office expenses and repair costs are among the information you must provide.

Massachusetts Financial Statement Schedule A CJD301A Step 4: Enter your total monthly expenses at the top of the second page.

Massachusetts Financial Statement Schedule A CJD301A Step 5: Subtract your expenses from your gross income, then divide the resulting number by 4.3. Enter the resulting number into the space for your weekly business income.

Massachusetts Financial Statement Schedule A CJD301A Step 7: If your business is seasonal, complete the chart in question two which asks you to list how much of a percentage of your annual income each month contributed and the expenses incurred during that period.

Massachusetts Financial Statement Schedule A CJD301A Step 8: Question three asks you to indicate whether your business accounts are managed according to the calendar or fiscal year.

Massachusetts Financial Statement Schedule A CJD301A Step 9: If your business is operated on a fiscal year business, give the starting and ending dates in response to question four.

Massachusetts Financial Statement Schedule A CJD301A Step 10: State your gross receipts for the last year from the time of completing of this form in question five and your gross expenses during that period in question six.

Massachusetts Financial Statement Schedule A CJD301A Step 11: If you make less than $75,000 a year, file this form with CJD301S. If you earn more, complete form CJD301L.
