Form CJD301S Financial Statement Short Form

 

INSTRUCTIONS: MASSACHUSETTS FINANCIAL STATEMENT (SHORT FORM) (Form CJD301S)

In Massachusetts divorce cases in which issues of alimony, child support, attorney's fees and division of property are, both parties may be required to complete a financial statement. This is the short form version of that statement, which can only be completed by those who make less than $75,000 a year. The document may be found on the website of the Massachusetts Courts system.

Massachusetts Financial Statement (Short Form) CJD301S Step 1: Enter the name of the plaintiff and petitioner.

Massachusetts Financial Statement (Short Form) CJD301S Step 2: In section one, enter all personal information requested, including your name, address, job, workplace address and health insurance provider, if any.

Massachusetts Financial Statement (Short Form) CJD301S Step 3: In section two, list income from all sources. In addition to weekly salary and wages, these include government-administered benefits. Enter the total in line 2r.

Massachusetts Financial Statement (Short Form) CJD301S Step 4: In section three, list all itemized deductions from your gross income. Add the total in line 3f and subtract it from line 2r to determine your weekly adjusted net income. Enter that number in section four.

Massachusetts Financial Statement (Short Form) CJD301S Step 5: In section five, list all other deductions, such as funds withheld for a retirement plan. Subtract this total from line 4 to determine your net income and enter the number in section six.

Massachusetts Financial Statement (Short Form) CJD301S Step 6: In section seven, enter your gross yearly income from the last year and indicate how many years you have been paying into the Social Security program.

Massachusetts Financial Statement (Short Form) CJD301S Step 8: In section nine, list your attorney and legal fees, as well as the total cost you expect to incur during the legal process.

Massachusetts Financial Statement (Short Form) CJD301S Step 9: In section ten, detail all assets, including real estate, motor vehicles, pensions and retirement funds, life insurance and bank accounts. Include the current values.

Massachusetts Financial Statement (Short Form) CJD301S Step 10: In section eleven list all liabilities, along with the amount owed and the name of the creditor. Sign and date the form.
