INSTRUCTIONS: ALASKA MARRIAGE LICENSE APPLICATION
In order to marry in Alaska, a bride and groom must first submit a marriage license. In the cities of Fairbanks, Anchorage and Juneau, an application for a marriage license must be submitted to those cities' branches of the Bureau Of Vital Statistics. Residents of other cities should submit the application to their nearest court. Both the bride and groom must be present to complete this application before a licensed officer. Once submitted, a three day waiting period must end before the license can be picked up.
Alaska Marriage License Application Step 1: The groom should enter their information on the left. Enter your full name, address and phone number, as well as your date and place of birth. Make sure to include the country of your birth.
Alaska Marriage License Application Step 2: The bride should enter all of this same information on the right side of the page. Additionally, enter your maiden name where indicated.
Alaska Marriage License Application Step 3: Both bride and groom must separately check "yes" or "no" to the question regarding whether they have any blood relationship. Note that Alaska law prohibits relatives who are more closely related than first cousins from marriage.
Alaska Marriage License Application Step 4: Check "yes" or "no" individually to the question regarding whether you know of any legal reason why the wedding should not be allowed to take place.
Alaska Marriage License Application Step 5: At the bottom of the page, both bride and groom should separately note what number this marriage will be for them. If married previously, indicate with a checkmark next to the appropriate box whether your previous marriage ended in death, dissolution, annulment or divorce.
Alaska Marriage License Application Step 6: The second page is optional and only for the use of those who have been previously married. Both bride and groom should enter the date and location of every previous wedding, their spouse's name, the way each marriage ended, and where and when the divorce was finalized.
Alaska Marriage License Application Step 7: If the bride and groom are under 18, their guardian's written consent must be provided at this time.
Alaska Marriage License Application Step 8: The presiding licensing officer will swear the bride and groom in. After this is done, you can sign the document.
