If you are not represented by a lawyer, instead provide your name, address, phone number, fax number and email address.

In the first column, enter the name of each heir. In the second column, enter the age of each heir who is a minor. In the third column, enter the address of each heir. In the fourth column, enter each heir's share of intestate property (if applicable). In the last column, enter each heir's relationship to the decedent.
