This form is required in forming a limited liability company within the state of Maryland. The state recommends working with an attorney, accountant or other professional while completing this form.
“Articles of Organization for Limited Liability Company Step 1”
Enter the name of the limited liability company on line 1. State law requires the name to contain one of the following designations: Limited Liability Company, L.L.C, LLC, L.C., or LC. The name cannot mislead the public by appearing similar to other company names in the state.

Indicate the main purpose on the limited liability company on line 2. You only need to provide one or two sentences.

Provide the street address for the limited liability company’s main office in Maryland. Do not provide a PO Box.

Enter the name of the resident agent on line 4. An individual or business entity qualifies as a resident agent as long as the individual is an adult resident of the state or the business is a corporation or LLC in the state of Maryland.

Any authorized person under the limited liability company can sign on line 5. The resident agent needs to sign on line 6 as well. Make sure to provide a return address on line 7 for any correspondence during the filing process.

The minimum filing fee for this form is $100.00. If you request a Certified Copy, add $20.00 and $1.00 for every page to the filing fee.
