INSTRUCTIONS: ARKANSAS REQUEST FOR CHILD SUPPORT SERVICES
Arkansas custodial parents who have not been paid part or all of court-ordered child support payments can use the request form discussed in this article to request enforcement assistance from the Office of Child Support Enforcement. This document can be obtained as part of a packet on the website maintained by the Arkansas Department of Finance and Administration.

Arkansas Request For Child Support Services Step 2: The second through eleventh pages contain instructions and resources for further assistance.
Arkansas Request For Child Support Services Step 3: The actual request form begins on the twelfth page. The first page requires you to document all children for whom you are seeking services. In the first column, enter the name of each child.
Arkansas Request For Child Support Services Step 4: In the second column, enter the sex of each child.
Arkansas Request For Child Support Services Step 5: In the third column, enter the date of birth of each child.
Arkansas Request For Child Support Services Step 6: In the fourth column, enter the Social Security number of each child.
Arkansas Request For Child Support Services Step 7: In the fifth column, enter the county and state of birth of each child.
Arkansas Request For Child Support Services Step 8: In the sixth column, indicate whether or not a paternity agreement was signed at the hospital.
Arkansas Request For Child Support Services Step 9: The second page concerns information about the person applying. Provide all information required about yourself.
Arkansas Request For Child Support Services Step 10: The third page concerns the noncustodial parent. Provide all information required about them.
Arkansas Request For Child Support Services Step 11: The fourth page concerns your legal status with the noncustodial parent, medical insurance and asks you to indicate how you learned about the services of the Office of Child Support Enforcement.

Arkansas Request For Child Support Services Step 13: Sign and date the bottom of the sixth page. However if you are a recipient of cash assistance or receive Medicaid for yourself, do not sign and date this page or submit it.
