INSTRUCTIONS: KENTUCKY CERTIFICATE OF QUALIFICATION (Form AOC-807)

 

 

The form discussed in this article is completed by the clerk and deputy clerk of a Kentucky court to certify the qualification of a fiduciary involved in a probate case. As part of the process during which this form is submitted, you may be required to submit a fiduciary bond (form AOC-825). Both of these forms can be obtained from the website maintained by the Kentucky Court of Justice. When completing form AOC-807, you may choose to complete all portions on a computer, which includes the option of selecting your county from a pull-down menu. You are also permitted to print out the form and complete it by hand.
