Form CT PC-901 Application for Change of Name (Adult)

INSTRUCTIONS: APPLICATION FOR CHANGE OF NAME (Adult) PC-901

 

The necessary forms need filed at the Probate Court in the county where the applicant currently resides.  An Affidavit Form PC-910 is also required along with this form.  Make sure to bring two forms of identification with your while filing these forms. 

“Application for Change of Name (Adult) PC-901 Step 1”

On the first lines of the form, provide your current legal name and the date of filing the application.  In the lines below the first section, provide your current legal name and then the name you wish to change your current name to.

In the next set of lines, provide your social security number and the name that appears on your birth certificate.

On the next two lines, provide your present address and your main telephone number with area code.  On the next line, specify how long you’ve lived at the current address.

If there is a non-petitioning spouse, provide their full legal name and current address.  If there is a co-petitioning spouse, have them fill out the next section of the document.

In the third section, provide specific reasons why you’re wishing to change your name.  If you try to change your name and cause fraud or deceit, your actions are punishable by law.

At the bottom of the form, the petitioner needs to provide their signature and date of signature.  If there is a co-petitioner, they need to provide their signature as well.

After completing the forms, make copies and file them with the Court Clerk.  There will be a filing fee of $150.00.  If you are unable to pay the fee, you will have to fill out Form PC-184.

A notice of the hearing is sent to all interested parties before the hearing takes place.  Additionally, a notice of the application is forward to the Department of Public Safety before the hearing to make sure the applicant is not on the Sex Offender Registry.

INSTRUCTIONS: IA NAME CHANGE PETITION

 

This form needs filed in the District Court of the county where the petitioner currently resides.  Filing fees will range from county to county. 

“IA Name Change Petition Step 1”

In the heading, provide your county’s name and your current legal name.  The clerk will provide the case number.

In part 1, provide the petitioner’s current and full legal name along with the county of their residence.

In part 2, provide the petitioner’s height in feet and inches, weight, hair color, and eye color.

 In part 3, provide the petitioner’s race, gender, city and state of birth, and date of birth including month, day, and year.

In part 4, provide all real estate that the petitioner owns in the state of Iowa.

In part 5, provide the address where the petitioner currently resides.  Also include addresses where the petitioner has lived in the past 5 years on the lines provided.

In part 6, check the appropriate box indicating if the petitioner is married or unmarried.  If the petitioner is married and wants to change their name, they must give legal notice to the spouse.

In part 7, state why the petitioner wants to change their name.  Then, in part 9, provide the petitioner current legal name on the first line and the proposed change of name on the second line.

Sign the first line for the petitioner’s signature.  The second signature must be performed in front of Notary Public.  Attach a certified copy of the petitioner’s birth certificate to the Petition and make a copy of the petition as well.  The copy will be filed with the Division for Records and Statistics under the Iowa Department of Public Health.

If the change of name is granted, the Decree will be available 30 days after the filing of the petition.  The Decree will provide information about the petitioner and any minor children that may be affected by the change of name.

Form MN NAM202 Application for Name Change of a Minor

INSTRUCTIONS: APPLICATION FOR NAME CHANGE OF A MINOR (MN NAM202)

 

All forms need filed with the District Court in the County where the minor child currently resides.  Apart from the Application, a Proposed Order Granted Change of a Minor (NAM107) and Criminal History Check Release (NAM103) are required.  The criminal check is only required if the child is 10 or older.

Complete the heading of the form including the county, the current name of the minor, and the new name of the minor.  Leave the court information blank.

List the current addresses of the minors, and check the box indicating you are a parent, legal guardian, or next of kin.  The list the current legal names of the children and their dates of birth.

In part 5 and 6, provide as much information as possible.  If the name and and/or address of the other parent is unknown, leave the space blank and check the appropriate box.

In part 9, provide a legal description of property the minor child has a claim, interest or lien.  If you need help filling out this form, ask for the assistance of an attorney.

The applicant needs to provide a signature in both designated places on the form.  If the minor is 14 years of age or older, they need to sign the form as well.  Sign the form in front of Notary Public.

If the address of the non-applicant parent is known, you need to ask an attorney of steps you should take because every county is different.  Some counties require mailed notice indicating the date, time, and place of the hearing, and other counties require personal service.

During the hearing, bring two adult witnesses who’ve known the minor for at least one year.  Only bring one witness if both parents attend the hearing.  Bring the child’s birth certificate.  If the name change is approved, the judge will sign an Order Granting Name Change of Minor.
