Form CA NC-100 Petition For Change Of Name

INSTRUCTIONS: PETITION FOR CHANGE OF NAME CA NC-100

 

This form needs filed in the superior court of the county where the petitioner currently resides.  The form can be used to change the petitioner’s name and the names of other, like children who are minors. 

“Petition for Change of Name CA NC-100 Step 1”

The following forms are also required along with this form:

·  Name and Information about the Person Whose Name is to be Changed (NC-110)

·  Order to Show Cause for Change of Name (NC-120)

·  Decree Changing Name (NC-130 or NC-130G for guardians)

If a guardian is filing for a change of name, the must also file NC-110G.

After completing all of the necessary forms, you’ll have to complete form CM-010 and submit the form along with two copies of the petitioner to the clerk.  There will be a filing fee, and fees will range from district to district.  Ask the clerk about waiving or deferring the fee.

Set up a hearing with the clerk for a minimum of six weeks in the future.  You’ll have to take the Order to Show Cause form to the clerk, and they will provide you with a date and location for the hearing.

In most cases, the petitioner will be required to publish the Order to Show Cause with a local newspaper once a week for four consecutive weeks before the hearing.  Once the newspaper has published the notice, the newspaper will have to provide the clerk with a Proof of Publication.

If there is no objection filed with the court two days before the hearing, the court will most likely grant the change of name without a hearing.  If a hearing is required, bring copies of all documents with you to the hearing.

If the petitioner is requesting a name change for a minor, regard section 8a-d on the form’s instructions.

After the name change is approved, you can begin changing your records.  If you need to change your birth certificate, contact the California Department of Health Services, Office of Vital Records.

Form CT PC-900 Application for Change of Name (Minor)

INSTRUCTIONS: APPLICATION FOR CHANGE OF NAME (Minor) PC-900

 

If a change of name is being requested for an adoption, the change will usually occur in the adoption process.  An Affidavit (PC-910) is required along with the filing of this application. 

“Application for Change of Name (Minor) PC-900 Step 1”

Provide the name of the County in the first line along with the district number.  On the next line, provide the current legal name of the minor along with the date of the application.

In the next section, check the appropriate box and provide the full legal name of the petitioner along with the telephone and current address.  If there is a non-petitioner parent or guardian, provide their name, current address and telephone number in the space provided below.

Below the information for the non-petitioning parent or guardian, check all of the appropriate boxes.  Then explain why the non-petitioning parent or guardian is not participating in the petition.

If there is a co-petitioner, provide the necessary information in the next section.

The next two sections list the current legal name of the minor, requested name of the minor, their present address, their date and place of birth, and their social security number.  If more sections are needed, another form will need filled out.

In the next section, the petitioner(s) need to provide specific reasons why they are requesting a change of name for the minor.

At the bottom of the application, the petitioner(s) needs to provide their signature and date of signature on the lines provided.  Make sure to sign the form in front of the Clerk.

Make copies of the forms and file the forms with the Clerk.  There will be a filing fee of $150.00.  Fill out Form PC-184 if you cannot afford the filing fees.

The petitioner(s) will be provided with the date, time and location of the hearing.  All interested parties will be informed of the hearing.
