Form ME CN-1 Petition for Change of Name (Adult)

INSTRUCTIONS: PETITION FOR CHANGE OF NAME (Adult) CN-1

 

This form needs filed in the Probate Court where the petitioner resides.  A certified copy of a birth certificate is required along with the form, as well as a divorce decree in applicable cases.  Ask the Clerk if affidavits and other supporting documents are required for your county. 

“Petition for Change of Name (Adult) CN-1 Step 1”

In the heading of the form, select your county and provide your full legal name.  If an attorney is helping you fill out the form, provide their name, contact information, and Maine Bar registration number. 

“Petition for Change of Name (Adult) CN-1 Step 2”

In part 1, provide the current legal name of the petitioner.  In part 2, provide the address and telephone number of the petitioner.  If the current address of the petitioner is different from the mailing address, provide the current address as well.

In part 3, provide the full legal name of the petitioner again.  In part 4, provide the requested change of name, including a middle name if applicable.

In part 5, provide reasons why the petitioner choose to change their name.

Date and sign the bottom of the form.  If the petitioner is being represented by an attorney, the attorney needs to sign the form on the appropriate line as well.

Once the petition is filed with the Clerk, the petition will be published in a local newspaper.  The filing fees include publication costs so the petitioner will not have to pay additional publishing fees.  If Notice is required, Form N-103 is filed with the Clerk.  If anyone needs Notice, they must be served at least 14 days before the hearing

Ask the clerk about your requirements for attending the hearing.  Some counties require the petitioner to attend the hearing while others do not.

You’ll receive VS-14 after the hearing if the name change was approved.  Obtain certified copies of this form to provide proof for changing your records.  If you need to change your birth certificate, a $20.00 fee needs made payable to the Treasurer, State of Maine.
