Form Intention of Marriage

 

INSTRUCTIONS: STATE OF MAINE INTENTION OF MARRIAGE FORM

To receive a marriage license, couples in the state of Maine must submit an intention of marriage form along with a separate form recording both parties' Social Security numbers. This documentation can be obtained from the website of the state of Maine. The document must be signed in the presence of a notary clerk or municipal clerk.

State Of Maine Intention of Marriage Form Step 1: The groom should enter their full name, including any suffix.

State Of Maine Intention of Marriage Form Step 2: The groom should enter their complete address and their age.

State Of Maine Intention of Marriage Form Step 3: The groom should enter the full name and birthplace of both parents.

State Of Maine Intention of Marriage Form Step 4: Below the section reserved for the groom, the bride should enter all her information. The only additional requirement is that she include her current last name and, if different, her maiden name.

If previously married, list the name of your last spouse and the date on which the marriage ended. Specify with a checkmark whether the marriage ended by death, annulment or divorce.

INFORMATION: WAYNE COUNTY MARRIAGE LICENSE APPLICATION

If you wish to get married in Wayne County, Michigan, at least one of the parties must appear in person at the Coleman A. Young Municipal Center in Detroit to submit the application for a marriage license. Both parties must reside in Wayne County. If both parties reside elsewhere, this ceremony must be performed in Wayne County. This document can be obtained in advance from the website of the Wayne County Clerk.

Wayne County Marriage License Application Step 1: The groom should enter their information in the left column of the page as indicated, while the bride should enter her information in the right column. With the exception of requesting that the bride list her maiden name if applicable, there is no difference in the information requested of both parties.

Wayne County Marriage License Application Step 2: Both parties should enter their full names.

Wayne County Marriage License Application Step 3: Both parties should enter their full address, including the county where they currently live.

Wayne County Marriage License Application Step 4: Both parties should list their age and the date of birth, as well as the place where they were born. A certified copy of their birth certificate must be presented when submitting this application.

Wayne County Marriage License Application Step 5: Both parties should their parents' full names and places of birth. In the case of the mother, the maiden name should be entered along with the full name.
