INSTRUCTIONS: HAWAII MARRIAGE LICENSE APPLICATION

In Hawaii, couples who plan on being married must submit an application for a marriage license in person before a marriage license agent. You may submit an application for a marriage license online to save time, but the bride and groom must still appear in person to obtain the license. This article discusses filling out the Hawaii marriage license application in various offices of the Department of Health located on the islands. This form can be viewed on Hawaii's Department of Health website.

Hawaii Marriage License Application Step 1: The groom should complete the top half of the page. Enter your full name and Social Security number where requested.

Hawaii Marriage License Application Step 2: Enter the full date of birth and the location.

Hawaii Marriage License Application Step 3: Enter the full address of the place where you most often live.

Hawaii Marriage License Application Step 4: Enter the full name and place of birth of both parents. You may choose not to answer whether each parent is alive or not.

Hawaii Marriage License Application Step 6: The section below these bride and groom information sections is to be completed jointly. Note the blood relation of the couple, if any. Provide the county name where the ceremony will take place and the name of the official who will be officiating.

Hawaii Marriage License Application Step 7: Enter a mailing address at which you can be contacted after marriage, along with an email address and home phone number. Entering a work phone number is optional.

Hawaii Marriage License Application Step 8: Check "yes" or "no" to the question regarding whether you you wish to have notice of your marriage published online or in newspapers.

Hawaii Marriage License Application Step 9: Enter the middle and last name both bride and groom intend to use after marriage.

Hawaii Marriage License Application Step 10: Where indicated, enter what number this marriage will be for you and specify how the previous marriage, if any, ended.

INSTRUCTIONS: MONTANA MARRIAGE APPLICATION

Citizens of the state of Montana must submit an application for a marriage license before they can perform this ceremony. There is one Montana Marriage Application which is valid statewide. This document can be downloaded from the website of Gallatin County, Montana. Both parties must be present to submit their application to the appropriate district court.

Montana Marriage Application Step 1: The groom should begin filling out the application in box five. The spaces above are reserved for the clerk of court in their area.

Montana Marriage Application Step 2: The groom should enter his full name and address.

Montana Marriage Application Step 3: The groom should enter his Social Security number, place and date of birth, and his current age.

Montana Marriage Application Step 4: The groom should enter the full name, current address and birthplace of both parents.

Montana Marriage Application Step 5: The groom should enter the description of their race they feel is most applicable.

Montana Marriage Application Step 6: The groom should specify the last grade they completed from one through twelve. If any college education was completed, enter a number from one to four to indicate how many years towards a bachelor's degree were completed. If you went on to graduate studies, enter "5+."

Montana Marriage Application Step 7: Enter the number of this marriage. If previously married, provide the name of your previous spouse, the way in which the marriage ended, and the location and date on which the marriage was terminated by death, divorce or annulment.

Montana Marriage Application Step 8: The same information is requested of the bride, as well as her maiden name, if different from current last name.

Montana Marriage Application Step 9: Lines 27 through 31b, 34 and 37-39 will be completed by the clerk of District Court. Do not enter any information here.

Montana Marriage Application Step 10: Note in line 32 any relation between the two spouses and if so what it consists of.

Montana Marriage Application Step 11: If a previous marriage application has been rejected, note this in line 33a and indicate the reason why.

Montana Marriage Application Step 12: Enter the address you plan to live at after the ceremony in line 35a.

INSTRUCTIONS: STATE OF ALABAMA MARRIAGE APPLICATION, GROOM

In some counties in Alabama, a bride and groom who wish to marry must submit separate marriage license applications rather than filing a joint application for marriage. The application discussed here is for a groom and can be obtained from several different purposes. This form in particular can be obtained from the website of the Madison County Probate Judge's Office.

State of Alabama Marriage Application, Groom Step 1: Enter your first, middle and last name on the separate lines where indicated.

State of Alabama Marriage Application, Groom Step 2: Under the section marked "Current Residence," enter your full address, including telephone number, street address, city name, county, state and zip code. There is a question asking whether you live within city limits. Circle "yes" or "no."

State of Alabama Marriage Application, Groom Step 3: Under "Birth Information," write the month, date and year of your birth. Circle the form of identification you have to verify your age. The options listed are "Military ID," "Driver's License," "P+V," or "birth certificate."

State of Alabama Marriage Application, Groom Step 4: Under "Personal Information," circle "Black," "White," "Asian," "Hispanic" or "American Indian," whichever best describes your race. Enter your Social Security number where indicated.

State of Alabama Marriage Application, Groom Step 5: Under "Education level," enter a number from 1 to 16 describing the last grade you completed. Numbers 1 through 12 refer to elementary, middle and high school, while 13 to 16 refer to the four years it takes to obtain a bachelor's degree.

State of Alabama Marriage Application, Groom Step 7: Enter the number that this marriage will be. If you have been previously married, indicate whether your last marriage ended in divorce, annulment, death or dissolution by circling the appropriate option.

State of Alabama Marriage Application, Groom Step 8: If you are under 18 years of age, list the name and city and state of your guardian. This person must give consent for the wedding to take place.
