INSTRUCTIONS: CONTACT PREFERENCE AND MEDICAL HISTORY FORM FOR BIRTH PARENT(S) OF ADOPTED CHILDREN BORN IN ALABAMA (Forms HS-86/HS-87)

 

 

Birth parents who give up their child for adoption in Alabama use a form HS-86 to indicate their preference about allowing their child to view their contact preference and medical history form, which is form HS-87. Both documents are part of one package available on the website of the Alabama Department of Public Health.

INSTRUCTIONS: STATE OF ALABAMA MARRIAGE APPLICATION, GROOM

In some counties in Alabama, a bride and groom who wish to marry must submit separate marriage license applications rather than filing a joint application for marriage. The application discussed here is for a groom and can be obtained from several different purposes. This form in particular can be obtained from the website of the Madison County Probate Judge's Office.

State of Alabama Marriage Application, Groom Step 1: Enter your first, middle and last name on the separate lines where indicated.

State of Alabama Marriage Application, Groom Step 2: Under the section marked "Current Residence," enter your full address, including telephone number, street address, city name, county, state and zip code. There is a question asking whether you live within city limits. Circle "yes" or "no."

State of Alabama Marriage Application, Groom Step 3: Under "Birth Information," write the month, date and year of your birth. Circle the form of identification you have to verify your age. The options listed are "Military ID," "Driver's License," "P+V," or "birth certificate."

State of Alabama Marriage Application, Groom Step 4: Under "Personal Information," circle "Black," "White," "Asian," "Hispanic" or "American Indian," whichever best describes your race. Enter your Social Security number where indicated.

State of Alabama Marriage Application, Groom Step 5: Under "Education level," enter a number from 1 to 16 describing the last grade you completed. Numbers 1 through 12 refer to elementary, middle and high school, while 13 to 16 refer to the four years it takes to obtain a bachelor's degree.

State of Alabama Marriage Application, Groom Step 7: Enter the number that this marriage will be. If you have been previously married, indicate whether your last marriage ended in divorce, annulment, death or dissolution by circling the appropriate option.

State of Alabama Marriage Application, Groom Step 8: If you are under 18 years of age, list the name and city and state of your guardian. This person must give consent for the wedding to take place.

INSTRUCTIONS: FLORIDA MARRIAGE LICENSE INFORMATION BY MAIL

 

People who wish to get married in the state of Florida may apply for a license to do so by mail. Every county has its own form and procedures established, which are all roughly the same. The document discussed here is from Brevard County and can be obtained from that area's circuit court website. Note that this is not an actual application for a marriage license, but simply a way to provide information that will be transferred to a license application. This form can be applied by mail or email.

 

Florida Marriage License Information By Mail Step 1: The groom should enter their full name, including any suffix.

 

Florida Marriage License Information By Mail Step 2: Circle the race which best describes you. If you do not fall under "White," "Indian," "Native American," "Oriental/Asian," "Pacific Islander" or "White," circle "Other."
