INSTRUCTIONS: PETITION FOR GUARDIANSHIP OF MINOR NHJB-2024-F

This form needs filed in the County Probate Court where the minor currently resides. 

“Petitioner for Guardianship of Minor NHJB-2024-F Step 1”

Parts 1, 2, 3, 4 require the petitioner to provide legal names, contact information, and more for the petitioner(s), the attorney for the petitioner, and the minor. 

“Petitioner for Guardianship of Minor NHJB-2024-F Step 2”

Part 5 asks for the names, date of birth, address, and telephone numbers of the mother and father.  If the mother and father are deceased, the petitioner needs to attach a list of names and addresses for the minor’s grandparents, adult siblings, aunts and uncles along with a death certificate of the parents.

Part 6 requires personal and contact information for the proposed guardian(s). 

“Petitioner for Guardianship of Minor NHJB-2024-F Step 4”

Part 7 requires the petitioner to list information about the minor’s residence and who they lived with for the last 5 years.  Form NHJB-2656-FPS is required if you need additional space.  Check the box that applies in part 8 and provide any necessary information.

Part 9 and 10 ask about court proceedings.  Check the appropriate boxes in parts 9 and 10 and provide the name of the court, state, case number, and type of court case.  Only check the box in part 11 if it applies.

Parts 12 and 13 require the petitioner to provide information about a person with custody of the minor for at least 60 days before the hearing, as well as any nominated guardians listed within the will of a deceased parent.

If there are other people living with the minor except the proposed guardian, complete part 14 and have the person complete a Criminal Record Release Authorization form and a DHHS Record Release Authorization form.

For parts 15 through 22, check the appropriate boxes and attach any necessary forms.

Parts 23 and 24 request temporary or permanent order and provide additional information why the guardianship meets the best interests of the child. 

“Petitioner for Guardianship of Minor NHJB-2024-F Step 10”

The petitioners need to provide a signature in front of Notary Public.
