You may represent yourself in Oregon family court cases concerning minor children. This document discusses form 3A, which can be filed in cases involving disputes over custody, visitation rights and child support payments. The form is available on the website of the Oregon courts system.

Oregon Petition For Custody And Parenting Time (And Child Support) 3A Step 1: Enter the name of the county you are appearing in, the case number you have been assigned and the name of the minor child or children in question.

Oregon Petition For Custody And Parenting Time (And Child Support) 3A Step 2: In section one, indicate which parties are the petitioner and respondent. Give the names and birth dates of all children.

In section six, give your wishes regarding child and medical support payments. In section seven, state which party should be responsible for maintaining life insurance for the benefit of their children while support payments are being made. Note any additional requests in section eight.

Oregon Petition For Custody And Parenting Time (And Child Support) 3A Step 9: In section eleven, indicate whether you prepared this document yourself or whether you paid someone else to assist you. If so, give their name. Section twelve concerns the residency status of all parties involved.
