INSTRUCTIONS: REQUEST TO ADD FATHER TO CHILD'S ALABAMA BIRTH CERTIFICATE (Form ADPH-HS-75)
To request that a father's name be added to a child's Alabama birth certificate, a form ADPH-HS-75 should be filed. This document can be obtained from the website maintained by the Alabama Department of Public Health.
