INSTRUCTIONS: POWER OF ATTORNEY – ODOMETER DISCLOSURE FORM (Form 5047)

 

Any vehicle which is less than 10 years old and/or under 16,000 pounds in weight must provide an odometer disclosure form. When the original title has been lost, destroyed or otherwise made unavailable, the person selling the vehicle must complete a Power of Attorney – Odometer Form. This is Form 5047, which can be obtained from the website of the South Carolina Department of Motor Vehicles.
 

Power of Attorney – Odometer Disclosure Form 5047 Step 1: Either the seller or dealer may enter the year, make, model, body type and vehicle identification number at the top of the form.

 

Power of Attorney – Odometer Disclosure Form 5047 Step 2: The seller should enter their name, the name of the person buying the vehicle, and enter the reading of the odometer. Check the box indicating the mileage indicated is beyond the vehicle's mechanical capabilities. If you are aware that the odometer mileage reading is not accurate, check the box next to this statement.

 

Power of Attorney – Odometer Disclosure Form 5047 Step 3: The seller should sign and print their name, record the date and provide their address. The buyer should sign and print their date.

 

Power of Attorney – Odometer Disclosure Form 5047 Step 4: Part B of this form will be completed when the vehicle is sold to a new owner. The seller should check the current odometer reading, enter the number where indicated and once again check either of the statements regarding inaccurate readings if appropriate. Both buyer and seller should sign and print their names, as well as providing the date and their addresses. If the person selling the vehicle is part of a dealership, their business name should be listed as well.

 

Power of Attorney – Odometer Disclosure Form 5047 Step 5: Part C of this form should be completed when a new title is obtained with the odometer reading from part B. The person selling the vehicle should enter the reading on the odometer, sign the form and provide their address.

 

Power of Attorney – Odometer Disclosure Form 5047 Step 6: The original copy of the form should be attached to the new title. One copy should be submitted to the South Carolina DMV. One copy should be kept by the seller for five years.
