INSTRUCTIONS: ARIZONA FOSTER ADOPTION AGREEMENT (Form FC-074)

When an Arizona resident agrees to adopt a foster child along with their spouse, they will complete a form FC-074. By signing this document, you are stating that you have received enough information concerning the background of the child and their current status and are willing to assume responsibility for a child who is not legally free for adoption and understand your responsibilities. This document can be obtained from the website maintained by the  Arizona Department of Economic Security.

Arizona Foster Adoption Agreement FC-074 Step 1: On the first blank line, enter the name of the child.

Arizona Foster Adoption Agreement FC-074 Step 2: On the second blank line, enter the month, date and year of the child's birth.

Arizona Foster Adoption Agreement FC-074 Step 3: On the third blank line, enter your full name.

Arizona Foster Adoption Agreement FC-074 Step 4: On the fourth blank line, the second adoptive foster parent should enter their name.

Arizona Foster Adoption Agreement FC-074 Step 5: On the fifth blank line, the first adoptive foster parent should provide their signature.

Arizona Foster Adoption Agreement FC-074 Step 6: On the sixth blank line, the first adoptive foster parent should enter the date.

Arizona Foster Adoption Agreement FC-074 Step 7: On the seventh blank line, the second adoptive foster parent should provide their signature.

Arizona Foster Adoption Agreement FC-074 Step 8: On the eighth blank line, the second adoptive foster parent should enter the date.

Arizona Foster Adoption Agreement FC-074 Step 9: On the ninth blank line, the child's worker should provide their signature.
