INSTRUCTIONS: WASHINGTON D.C. REPORT OF VISITOR

 

 

The form discussed in this article is used by someone appointed as a visitor in a Washington D.C. probate case to file their report. This document can be obtained from the website maintained by the District of Columbia Courts system.

 

Washington D.C. Report Of Visitor Step 1: Enter the name of the adult who is the subject of the conservatorship or guardianship on the first blank line.

 

Washington D.C. Report Of Visitor Step 2: On the next blank line, enter your name.

 

Washington D.C. Report Of Visitor Step 3: On the next blank line, enter the date of the order that appointed you as visitor.

 

Washington D.C. Report Of Visitor Step 4: On line IA, enter the date and place of your interview with the subject of the proceeding.

 

Washington D.C. Report Of Visitor Step 5: On line IB, indicate with a check mark whether the interview was oriented by time and place.

 

Washington D.C. Report Of Visitor Step 6: On line IC, provide a description of the physical appearance of the subject.

 

Washington D.C. Report Of Visitor Step 7: On line 1, indicate with a check mark whether the subject answered "yes" or "no" to the statement printed there. If no, provide an explanation.

 

Washington D.C. Report Of Visitor Step 8: On line 2, indicate with a check mark whether the subject answered "yes" or "no" to the statement printed there. If no, provide an explanation.

 

Washington D.C. Report Of Visitor Step 9: On line 3, indicate the subject's answers to all questions listed there by checking "yes" or "no" to all statements.

 

Washington D.C. Report Of Visitor Step 10: On line 4, enter the subject's listing of their closest family members, including their names, addresses and relationship to the subject.

 

Washington D.C. Report Of Visitor Step 11: On line 5, indicate the subject's response to all questions printed there by checking "yes" or "no" as appropriate.

 

Washington D.C. Report Of Visitor Step 12: On line 6, indicate the subject's response to all questions printed there by checking "yes" or "no" as appropriate.

 

Washington D.C. Report Of Visitor Step 13: On line 7, write who the subject said they would like to have help care for them. Complete the rest of the form as instructed.
